The services included in this project are related to .services loaded into HS/S as of 07/31/2002. While 1 understand that 1 am still responsible for maintaining documentation for the services reported to DPH through HSIC, 1 understand that by reassigning the services, 1 am transferring responsibility for payment of these claims to DPH for any future audits and/or paybacks related to these services. 1 understand that DPH will maintain and use/distribute these funds in accordance with all agreements by and between DPH, local health departments and DECs/CDSAs. 6.
